1. Failure to use eye protection that is approved by this facility may result in Permanente eye damage
2. Federal, State, and local regulations require you to use approved eyewear in the tanning systems
3. Overexposure to ultraviolet light causes burns
4. Abnormal skin sensitivity or burns may be caused by ultraviolet light to certain: foods, cosmetics or medications, including but not limited to: tranquilizers, birth control pills, diuretics, high blood pressure medications, and antibiotics
5. Any person using a prescription, or over the counter drugs should consult a physician before tanning
6. A person with skin, that always burns and never tans, should avoid the tanning devices
7. A person with a family, or past medical history f skin cancer should avoid the tanning devices
8. Pregnant women should consult a physician before using a tanning device
9. TanMan LLC, and, its designates, subsidiaries, beneficiaries, associates, parent company’s, vendors, distributors, affiliates and subsidiaries reserve the right to,at there sole discretion to charge the following fee’s, as agreed upon by you signing this agreement: 1.a NSF fee of up to $39.99 or the local legal maximum of your state of residence (which ever is less),2.A re-clear fee of 19.99 or the local legal maximum in the state of your residence (which ever is less) 3. If any debt associated with TanMan LLC, TanMan salon’s LLC, TanMan franchising LLC, MIH of Beverly Hill’s, AAC Investment Management LLC, MIH Telecom LLC, MIH Entertainment & Production’s LLC (AKA: MIHEP LLC), Salon Holding’s Internationally LLC, or any of there individual or cumulative, distributors, vendors, affiliates, associates, subsidiaries, or holding’s, is paid more than 3 calendar days after the scheduled due date, a 10% retroactive APR Will be charged, beginning and in correlation to the date that the product/service was originally received or contracted for( which ever was later).
10. In the event of default as described under 9 above TanMan LLC, and/or it’s affiliates or subsidiaries reserve the right to assessa twelve dollar per month delinquent account communication’s fee, plus a one time sixty nine dollar delinquent account set up fee, TanMan reseves the right at its sole discreation to charge a lesser amount at any time without effecting the validity of this agreement.
11. In the event of default or delinquency of any account held by you in correlation to TanMan LLC, it’s affiliates, or subsidiaries, you agree to allow TanMan LLC, it’s affiliates, designates, and subsidiaries to draft any account they may have “on file” until all accounts are considered current, and up to date regardless of your awareness of TanMan LLC, it’s affiliates and designates, having such account’s on file.
12. TanMan reserves the right to suspend and/or cancel any membership or account without out notice, refund, or warning there of, for any reason, without recourse of the original buyer.
13 TanMan reserves the right to refuse service to any one at any time.
14 TanMan LLC, its affiliates and subsidiaries, reserves the right to change location/hour/policy’s/procedures/ and/or equipment, at any time without notice. I understand that TanMan reserves the right to discontinue any free parking services, without recourse.
15. In the event that I elect to use the name, combination of names or slogans of the following: TanMan, Tan Man, TanMan LLC, TanMan Tanning, TanMan salon, “Go to the man, to get your Tan”, www.TanManTanning.com or any combination or variation there of, I agree to pay TanMan LLC, via a statement (due within 10 days of receipt) or automatically via any payment form They have on file for me the following fees, per occurrence, per name/slogan of: Flyers/post cards/letters/ and non professional print (8 1/2 by 11 or less in size) base fee of two hundred dollars plus thirty five dollars per print, Flyers/post cards/letters and nonprofessional print larger than 8 ½ by 11 in size a base fee of one thousand dollars, and an additional fifty dollars per print, any professional print(including newspapers, magazines, internet magazines, chat rooms, web pages and/or blogs, and any other professional means, of written, or printed media) shall have a base fee of fifty thousand dollars, plus an additional two dollars per person, to which that media is in circulation to. All other forms of Media not described here in shall have a base fee of two thousand dollars, plus two dollars person that medium is in average circulation too, at the date to which circulation occurred. I understand TanMan reserves the right to charge a lesser amount at its sole discretion of any of the above at any time without effecting the validity of this agreement
16. I understand and agree that TanMan reserves the right to refuse service to any one, for any reason, even those with valid membership’s and/or packages, without refund, relief of obligation, or notice.
17. Severability: TanMan Reserves the right to sever any portion of this agreement at any time, if found that any portion of the agreement is, or has become unlawful/enforceable, however all remaining portion of the agreement shall remain valid and fully enforceable
18. Repeated exposure to ultraviolet rays may cause premature aging, skin damage and other conditions
I agree that I will fully comply with all instructions on the use of Tanning Systems in this salon, and I am using these tanning systems at my own risk. I herby hold harmless the Employees, owners, agents, and affiliates of this Salon, TanMan LLC, MIH of Beverly Hills and there affiliates from any injury or harm that may incur during, or as a result of using these facilities.
